Overcome your pain with our help!

Your First Physical Therapy Visit: Please arrive 15 minutes before your scheduled appointment to do the dreaded paperwork. Bring your insurance card, referral from the doctor and any other pertinent information. If this is an LI injury we need the date of injury, your claim number and information form from the doctor. If this is an auto injury we need the claim number, your auto insurance (regardless of who is at fault), the auto information from the person at fault, and your personal medical insurance information. Please bring a list of medications for our records. The forms are short answers and only take 10-15 minutes to fill out. If you need help, the front office receptionist is available.

●After you have completed your paperwork, you will be brought back to a private treatment room. The physical therapist will do a detailed evaluation. Upon completion of the evaluation the therapist will discuss their findings, discuss a treatment plan, number of treatments, goals, and answer any questions you might have. Your first visit may include application of modalities such as ice/heat, ultrasound, electrical stimulation, massage type techniques, and instruction in a home program.

Please don’t be afraid to ask questions!!! We are happy to answer any and all questions.



SCHEDULING: It is advised to schedule in advance to insure getting days and times that work best for you. We can schedule out a month in advance if needed. We do our best to accommodate your work schedule. Please cancel scheduled appointments at least 24 hours in advance. Your advance notice allows us to fill your spot with another patient.



LENGTH OF VISITS: The 1st visit usually takes longer so please allow 60-75 minutes. After the 1st visit allow 45-60 minutes. As you progress with your physical therapy exercise program, you may need to allow more time. Some patients spend 1-2 hours participating in a supervised gym exercise program. We will notify you when you need to allow more time as you advance. We pride ourselves with the quality of time the physical therapist spends with each patient. The physical therapist generally spend 30 minutes working with you on a individual basis before you participate in a supervised exercise program, designed by the therapist.



YOUR DOCTOR: In the state of Washington we can legally treat you without the consent of your doctor, but would prefer to have a referral from your doctor before starting therapy. Most insurance companies require a referral for payment. We like to keep in contact with you doctor and forward our evaluation, any progress notes, and a discharge summary upon completion of your physical therapy. We will request any medical reports that are pertinent to your case such as an MRI, X-Ray, surgical report, etc. If we have any questions or concerns regarding your care, we will discuss it with you and contact you MD. You are welcome to request copies of any information we acquire on your behalf, in addition to our reports.



INSURANCE: Our clinic accepts all major insurance companies with the exception of Group Health Cooperative and Essence Medicare Select.
